Here's a podcast from Brooke Warner, publisher at She Writes Press, as she interviews an author about this topic. Especially good for memoir writers! From Brooke Warner

A quick note for anyone writing about or planning to interview others. You'll most likely need a signed release form to be able to use the quotes and interviews in your book. This is standard book industry practice. So plan ahead and ask the person you may interview or quote if they would be willing to sign a release for you to use the material. This is more for books than for articles. With articles, it's generally a good idea to let the person you interviewed review what you plan to publish before sending it to the newspaper, magazine or blog. Happy Writing!
